Questions asked by Environmental Health Officer following a non-fatal accident on site

General Background

  1. Confirm that your Company was the Organisers of this exhibition.
      
  2. Outline your company’s background in organising exhibitions at venues such as (venue), including number of years experience, number of staff etc.
      
  3. Prior to your tenancy at (venue) what information did you receive from (venue) regarding Health & Safety during the exhibition.
      
  4. What did you understand your company’s responsibilities for the management of Health & Safety at the following stages of the exhibition to be:
      
    1. Build-up
    2. Open Period
    3. Breakdown
        
  5. Following discussions with House Management at (venue) it is apparent that they considered your Company to have full Health & Safety management responsibility over the whole exhibition, including the management of exhibitors, contractors, subcontractors you employed. What are your comments regarding this?

  13. Did you supply copies of the exhibition plans to (contractor) before they attended the exhibition? If so do these plans indicate aisle width?
